mactron on "[Plugin: WooCommerce - excelling eCommerce] Form on Product Page"

Hello,

I need to add form on the top of button "add to cart" ?

I try with "iphorm uniform builder" plugin but in this case i get two buttons (1. for form submit and 2. for add to cart)..
